Are hotel star ratings consistent across countries?

No, hotel star ratings are not consistent across countries. For example, a 3-star hotel in Italy may differ significantly from a 3-star in the US. Some countries—such as France, Spain, and Italy—have official rating systems, but most do not regulate star classifications.
